Closing-out your UTCOM certified CME Activity

Use the forms below to closeout CME activities that were certified by the UT College of Medicine (UTCOM) and to request issuance of the CME credit certificates/transcripts (AMA PRA Category 1 Credit™).  

CME Activity Closeout form - This form must be completed in order to close-out your UTCOM-certified CME activity and request issuance of the CME credit certificates/transcripts. 

CME Meeting Closeout form for CME activities with multiple meetings - This form must be completed in order to close-out an individual meeting/session (of a CME activity with multiple meetings) and request issuance of the CME credit certificates prior to the end of the activity (all meetings).

Resources for the Supporting Attachments

Required Attachment #1) Attendance Report - In order to have CME credits issued for your UTCOM-certified CME activity, you must provide us with an attendance report that summarizes the attendance for the activity.  The attendance report needs to be an Excel spreadsheet and needs to include (at a minimum) the first name, last name & number of hours attended for each attendee seeking CME credit.  If you want us to send the CME credit certificates directly to the attendees, your attendance report also needs to include the email address (if you want the certificates to be emailed to attendees) or the address, city, state & zipcode (if you want the certificates to be mailed to attendees) for each attendee. Use the templates below, or create & provide your own attendance report.

CME Attendance Report template for Single Session - This template provides a useful starting point for the attendance report that we need for a CME activity that only has one occurence. 

CME Attendance Report template for Multi-Session Series - This template provides a useful starting point for the attendance report that we need for a CME activity that has more than one occurence (i.e., regularly-scheduled series or mult-conference series).
